| ISSUE OF
PASSPORT TO MINORS
This page should be read in conjunction with the page on
our website headed Passport Application Form and Information.
In addition to the normal statutory requirements for an
Australian Passport, there are several additional steps that must be completed
for us to issue a Passport to a Minor. A "minor" is defined
in The Passports Act 2005 as any person under the age of 18 years. We
ask that you take a little extra time to read all the information enclosed
with the application form.
The additional statutory requirements which must be met
before we can issue either a new or replacement Passport to a minor are
as follows:
- Please note that where we need to sight a document,
we must see the original of that document. Unfortunately, we cannot
accept copies, even certified true copies, under any circumstances.
- The child's full Birth Certificate must accompany the
application and if the child was born outside of Australia, their Australian
Citizenship Certificate must be provided.
- The parent's Marriage Certificate must be provided,
if the mother is using her married name that does not appear on the
child's birth certificate.
- Proof of any change of name of the child and/or either
parent must accompany the application. The application must be lodged
in the current name being used by the child.
- Both Birth Parents (ie. those listed on the child's
Birth Certificate), must provide written consent to the issue of the
Passport by completing the Parental Consent section on page 4 of the
Application form. If there is any difficulty obtaining this consent
from either parent or if a grandparent or legally qualified person will
be lodging the application, please read the page on our Website headed

e-mail at Australia.org.pe.
- The Identifier section on Page 3 of the application
form must be completed in accordance with the instructions. For children
less than 1 years of age, the identifier is only required to have known
the child since birth, not the normal twelve months. Please ensure the
person completing this section is readily contactable by telephone.
- For photographs of new born babies, we suggest lying
the baby on a plain coloured (preferably white) rug/sheet, taking a
photo and printing two copies, which we will cut down to a passport
photo size. The child's eyes must be open, the photo must be as close
as possible to a front-on view of the babies' head and shoulders and
there should be no obvious obstruction.
Either parent may lodge the application in person and they
must bring along with them, proof of their current name and signature,
proof of the child's current name and the appropriate fee.
